Access the Passport for Care record any time on your iOS or Android device with the Passport for Care app! In the app, you can see your cancer treatment history and late effects you may experience due to your cancer treatment. You’ll also see posts about survivorship issues, a personal care plan, and a secure chat you can use to talk to your clinic.

New to Passport for Care?

Have you recently been enrolled in the Passport for Care by your survivorship clinic? Follow the instructions on your Activation Letter to activate your PFC app by scanning your personal QR code.

Already a Passport for Care member?

If you’ve already activated your PFC survivor account, you can sign in using the username and password you created when you activated.
